In which cases must an application be made?
For accreditation by DAkkS, submission of a formal application is always required. This applies to
- Initial accreditation
- Changes (modification / extension / reduction), suspension or withdrawal of accreditation requested by customer
No application is required for the surveillance of an existing accreditation.
How do I submit an application?
DAkkS forms are available for the efficient completion of the application process. The application for accreditation must include at least the following documents:
- We require a valid extract from the commercial register or other proof of the legal form of the applicant company.
- We require the completed application form for accreditation (72 FB 006_e).
Please note that the signatures on the application must be the same as the persons authorized to represent shown in the extract from the commercial register.
- A range of specific templates are available for the indication of the scope of accreditation, depending on the area of activity. An overview can be found in document List of the templates to apply for the scope of accreditation (72 FB 005_e).
If you already have an accreditation, you can instead build on the list of procedures found in the annex to your accreditation certificate (please mark changes).

Our clients outside the European Economic Area are requested to send the application by email to zab [at] dakks [dot] de.
